1. How do I sign up?

You can sign up for Leap by following these steps:

  1. Open the Leap app and tap the “Sign Up” button.

  2. Enter the activation code you were emailed from your doctor’s office.

  3. Confirm date of birth.

  4. Provide your email address, phone number, and set your password.

  5. Review and acknowledge the terms and conditions.

2. What do I do if my date of birth does not match the records?

You will receive the following message in this situation.

This means that the date of birth you entered does not match the date on your doctor’s record. Make sure to enter your correct date of birth. If this problem persists, contact your doctor’s office and check your birth date with them. They may have the wrong date on file and can resolve this issue.

3. What information do I need to sign up?

You will need the following information to sign up for Leap:

  1. The activation code you were emailed from your doctor’s office.

  2. Your date of birth.

  3. A verification code sent to your phone.

  4. Your phone number

  5. Your email address

4. Can I use my old patient portal credentials to sign up?

When you sign in to the old patient portal using your old patient portal credentials, the following page will appear.

It will prompt you to download the Leap app. To download the app, you can either scan the QR code or provide your phone number to receive a download link via text. Do not share the QR code or download link as they are user-specific and sharing it may compromise sensitive data.

5. How do I sign up with an invitation from my healthcare provider?

You can sign up for Leap by following these steps:

  1. Open the Leap app and tap the “Sign Up” button.

  2. Enter the activation code you were emailed from your provider’s office.

  3. Confirm date of birth.

  4. Provide your email address, phone number, and set your password.

  5. Review and acknowledge the terms and conditions.

6. What should I do if I don’t receive an activation code?

Contact your provider’s office and ask them if they are registered with Leap. If they are, ask them to email you an activation code. Currently, you may only sign up with Leap if your provider is registered with it.

7. What happens if the invitation code has expired?

If you use an old or expired invitation code, you will receive a notification indicating the code has expired. To proceed with the registration. You will need to contact your medical provider request them to generate a new invitation code. This process ensures only valid invitation codes are used for secure access, preventing unauthorized or delayed sign-ups.

8. What should I do if I forget my password?

You can easily reset your password by following these steps:

  1. Navigate to the login page and press “Forgot Password”.

  2. Enter the email associated with your Leap account.

  3. Enter the one-time-password (OTP) sent to your phone and email.

  4. Enter and confirm your new password.

9. Is there a difference between the app and web versions?

Leap is currently available as a mobile app; the web version will be launched soon.

10. Can I use the same account for both the app and web versions?

The web version of Leap has not been released yet. You will be able to use the same account for the app and web version once the web version is available.
